These cuts, part of the Trump administration's draft budget for fiscal year 2026, aim to eliminate several key bipartisan rural health support programs. Among the programs at risk are the State Office of Rural Health, the Medicare Rural Hospital Flexibility program (FLEX), and the Small Rural Hospital Improvement Program (SHIP). With 21 rural Colorado hospitals currently reporting negative profit margins, the loss of these programs could push them to the brink of closure.

The challenges facing rural health providers are manifold, including higher percentages of Medicare and Medicaid patients, a greater prevalence of chronic diseases, lower average incomes among patients, workforce shortages, and aging populations. The proposed budget cuts would exacerbate these challenges, potentially leaving entire communities without accessible medical services. Michelle Mills, CEO of CRHC, highlighted the broader economic implications, noting that every dollar invested in rural healthcare supports community vitality. According to the National Rural Health Association, hospitals nationwide support over 16 million jobs, with each hospital dollar generating $2.30 in additional economic activity.

DeeAnn Sierra, RN, from Rio Grande Hospital, underscored the potential devastating consequences of these cuts, particularly the loss of State Offices of Rural Health, which would severely impact rural healthcare providers' ability to serve their communities effectively. Last year, CRHC provided over 14,000 technical assistance touchpoints to rural hospitals and clinics across Colorado's 47 rural and frontier counties, serving 42 rural hospitals and more than 100 rural clinics. The organization's advocacy is critical to maintaining healthcare access in these remote regions.

The proposed budget cuts represent a significant threat to rural healthcare sustainability in Colorado and beyond. Mills has called for the full funding of SORH, Flex, and SHIP grants to ensure that rural healthcare facilities can continue to serve their communities sustainably.
